چکیده

Background Reverse total shoulder arthroplasty is an increasingly utilized treatment option for glenohumeral arthritis with massive, irreparable rotator cuff disruption associated with loss of glenohumeral stability and function. Several reverse shoulder implant designs from different manufacturers have been introduced to the market in recent years. Fixation of the glenoid base plate has been shown to be sensitive to the implant geometry and clinical complications related to the glenoid implant have been reported. The Zimmer® Trabecular MetalTM Reverse Shoulder prosthesis is designed to achieve both initial and long-term stability of the glenoid base plate implant through a unique combination of screw fixation and biological ingrowth. The purpose of this paper is to report on the testing that was conducted to quantitatively assess the effect of bone quality on the initial fixation of the Trabecular Metal Reverse Shoulder glenoid base plate implant.

Trabecular bone microarchitecture and characteristics in different regions of the glenoid.

Success of shoulder surgery depends on implant fixation to the glenoid trabecular bone. The purpose of this study was to evaluate the anatomic characteristics of the normal glenoid trabecular bone microarchitecture to help assist in implant design and provide data for finite element analyses. Eight cadavers without evidence of osteoarthritis were used. Glenoids were scanned with micro-computed ...

Mid-term results of a metal-backed glenoid component in total shoulder replacement.

Total shoulder replacement is a successful procedure for degenerative or some inflammatory diseases of the shoulder. However, fixation of the glenoid seems to be the main weakness with a high rate of loosening. The results using all-polyethylene components have been better than those using metal-backed components. We describe our experience with 35 consecutive total shoulder replacements using ...

Early Outcomes Following Metal-on-Metal Reverse Total Shoulder Arthroplasty in Patients Younger Than 50 Years.

Reverse total shoulder arthroplasty (TSA) is a useful intervention for older patients with glenohumeral arthritis and a deficient rotator cuff. However, as a semiconstrained prosthesis, conventional reverse TSA implanted in a young patient could fail over time secondary to polyethylene wear and subsequent osteolysis. A metal-on-metal prosthesis may avoid this type of failure. The purpose of thi...

Hip-inspired implant for revision of failed reverse shoulder arthroplasty with severe glenoid bone loss

BACKGROUND AND PURPOSE Glenoid reconstruction and inverted glenoid re-implantation is strongly advocated in revisions of failed reverse shoulder arthroplasty (RSA). Nevertheless, severe glenoid deficiency may preclude glenoid reconstruction and may dictate less favorable solutions, such as conversion to hemiarthropasty or resection arthropasty. The CAD/CAM shoulder (Stanmore Implants, Elstree, ...

Use of a caged, bone ingrowth, glenoid implant in anatomic total shoulder arthroplasty technique and early results.

Shoulder arthroplasty represents one of the fastest growing orthopaedic procedures in the USA. In comparison to hemiarthroplasty, total shoulder arthroplasty has higher patient satisfaction and lower revision rates. Aseptic glenoid loosening remains a concern for longevity of total shoulder arthroplasties. Cement fixation on the glenoid side represents the primary and only mode of fixation in t...
